Consumer Health Data Privacy Notice
Effective: September 27, 2026
This Consumer Health Data Privacy Notice applies to residents of Washington, Nevada, and Connecticut. It supplements our Privacy Policy and explains how Klevahealth Inc. DBA Reprise Health ("Reprise," "we," "us") handles consumer health data under the Washington My Health My Data Act, Nevada SB 370, and the Connecticut Data Privacy Act.
What is consumer health data
Consumer health data is personal information that is linked or reasonably linkable to you and identifies your past, present, or future physical or mental health status. It does not include de-identified, aggregated, or publicly available information.
Consumer health data we collect
- Products you buy or view that could suggest a health goal (e.g., sleep, stress, focus, or digestion support)
- Health or wellness information you share with us, e.g., in quiz or survey answers, reviews, support messages, or reasons for a refund request
- HSA/FSA eligibility information related to a purchase made through Flex Technology Co ("Flex")
Why we collect it
We collect and use consumer health data only as reasonably necessary to:
- Process, ship, and support your orders, subscriptions, and refunds
- Respond to your questions and feedback
- Improve our products and Site
- Prevent fraud and meet legal obligations
We ask for your consent before collecting or using consumer health data for any other purpose.
Sources
We collect it directly from you, automatically when you use the Site, and from service providers such as our e-commerce platform and HSA/FSA partner.
Who we share it with
- Service providers that process data on our behalf under contract (e.g., e-commerce hosting, fulfillment, customer support, HSA/FSA payment processing)
- Third parties, only with your consent
- Government authorities, when required by law
- A buyer or successor, if our business is merged or sold
We do not sell consumer health data. We do not use geofencing to identify or track visitors, or to collect consumer health data, near any location that provides health care services.
Your rights
Depending on your state, you have the right to:
- Confirm whether we collect, share, or sell your consumer health data, and access it, including a list of the third parties and affiliates we've shared it with
- Correct inaccurate data (Connecticut)
- Delete your consumer health data
- Withdraw your consent at any time
- Opt out of targeted advertising (Connecticut)
- Not be discriminated against for exercising these rights
How to exercise your rights
Email support@reprisehealth.com with the subject line "Consumer Health Data Request." We'll verify your identity by matching information you provide, such as your email and a recent order number. We'll respond within 45 days. Where the law allows, we may extend that by another 45 days and will tell you if we do.
Appeals
If we deny your request, you can appeal by replying to our decision email with "Appeal" in the subject line. We'll respond within 45 days. If you're not satisfied with the result, you can contact the Attorney General in your state (Washington, Nevada, or Connecticut).
